The Office of Management and Budget (OMB) has given final approval to the latest versions of the Hospice Item Set (HIS) admission and discharge. All Medicare-certified hospices are required to submit an HIS-Admission record and a HIS-Discharge record for each patient admission occurring on or after July 1, 2014.

The collection instruments  are available on the  HIS web page in the “Downloads” section, and the data specifications, which detail the requirements for the submission of HIS records, are available on the HIS Technical web page. The final versions of the HIS estimate that completion of the HIS-Admission will take an average of 19 minutes, while completion of the HIS-Discharge is estimated to take 10 minutes.

The Centers for Medicare & Medicaid Services (CMS) conducted HIS training in February that is now posted on the HIS web page, along with additional reference materials.

The video recordings of the Data Collection Training for the Hospice Item Set (HIS), originally presented on February 4 and 5, 2014 are now available online here.

The Hospice Quality Reporting Program trainings are also available on YouTube. For the February 4 training video, please click here.
